You’ll need to save your documents in PDF format and collate multiple pages into ONE file. Each file must be 2MB or less.
Medical qualifications
You’ll need to provide the following:
- your medical degree, if you need an English translation you can find out about the requirements on the AHPRA website
- your Medical Board of Australia registration or renewal.
Proof of identity
You’ll need to provide proof of your identity.
If you’re a new applicant, you must provide 3 current identification documents from the list below. One must be a type of photographic ID with a signature and date of birth, preferably a current passport or Australian drivers licence.
For overseas applicants, local documents issued by equivalent overseas authorities may be accepted. When the name shown on the documents differs from the person’s current name, the person is to provide sufficient evidence of their name change (e.g. certified copy of marriage certificate).
Identity documents you can use include:
- current passport
- Australian drivers licence
- Australian student photo ID
- Australian proof of age card
- Australian citizenship certificate
- birth certificate
- health care card
- Medicare card
- pension concession card
- utility bill, for example water, gas, electricity
- working with children check (Blue Card).
If you’re a current employee, you must provide photographic ID, with a signature and date of birth. Preferably a current passport, Australian proof of age card or Australian drivers licence.
You must get your proof of identity documents certified by a Justice of the peace, commissioner of declarations, barrister, solicitor, or notary public. If you can’t, upload them anyway and when you get a position provide certified versions to your employing facility before you start work.
Curriculum Vitae (CV)
Our preferred template for CVs is the Queensland Health Medical practitioner CV template [DOC 319.5 KB]. It outlines all the items you need to include to submit an application with us.
If you’re using your own version, make sure it includes a 'verification statement’ and is signed and dated. For example, I verify that the information contained within this Curriculum Vitae is true and correct as at <insert date> and sign.
Vaccination evidence
You’ll also need to provide evidence of vaccination or non-susceptibility for any, or all, of the specified vaccine preventable diseases.
This includes pathology testing results for the following vaccine preventable diseases:
- hepatitis B
- measles
- mumps
- rubella
- varicella
- pertussis.
There is no time limitation or expiration for serology results. It is important to note that in the case of pertussis, there is no acceptable evidence of immunity. You must provide evidence that you’ve had a pertussis-containing vaccine within the past 10 years.
All new employees who will be working in clinical areas in a Queensland Health facility must be assessed for their risk of tuberculosis (TB) using the relevant TB risk assessment process. Download the Tuberculosis Risk Assessment Questionnaire for workers in clinical areas, complete and upload it to your online RMO application.
Criminal History check
If you're a new applicant, you’ll also need to complete the Queensland Health Criminal History Consent Form [PDF 235.33 KB]. You need to get this witnessed by someone over the age of 18.
Referee reports
You'll need 2 referees to complete and submit reports through the online RMO application. Find out about referees on the how to apply page.
Other documents required by overseas doctors
If you’re an overseas doctor there are some documents you may also need to provide, including:
- Australian Medical Council MCQ or Australian Medical Council Certificate
- visa documentation - visa letter
- your Department of Home Affairs VEVO Entitlements check
- other pre-employment checks.

Get your documents certified
Before you upload your supporting documents, you’ll need to get them certified. A justice of the peace, commissioner of declarations, barrister, solicitor, or notary public can certify them.
If you get a position, your employing hospital may ask you to provide your original documents or certified copies.
