Vaccine preventable diseases evidence
We have information below about the types of evidence we accept for vaccine preventable diseases.
You can talk to your hiring manager or placement coordinator if you're:
- not sure how to provide your evidence
- not able to meet the vaccination requirements for some reason.
If you're a student, you should should talk to your education provider.
Documenting your evidence
Use the following forms to document your evidence. You may only need to use one, depending on the requirements and documents you have.
VPD evidence certification form [PDF 83 KB] - ask your medical practitioner, registered nurse, or occupational health provider to fill in this form on your behalf.
VPD evidence form [PDF 103 KB] - use this form if you already have copies of acceptable forms of evidence. You must attach copies of the documents listed in the table below.

Types of evidence
The following table shows types of evidence we accept for the 6 main vaccine preventable diseases.
You need to provide one of the acceptable types of evidence for each disease.

Healthcare workers vaccination guideline
Our vaccination guide details the requirements and test results you'll need to included in your evidence.
Appendix 1 includes:
- the specific vaccination requirements you should find in your vaccine record book
- the pathology test results you need to show for each disease.
Appendix 2 has information about the:
- transmission, incubation, symptoms and consequences for each of the VPDs
- corresponding vaccines and their possible side effects.
